Who You Are
Client Centered and Trauma Informed
You approach this work with a deep respect for the dignity, autonomy, and lived experiences of the people you serve. You understand that care coordination is not simply about connecting clients to resources; it is about building trust, navigating systems that were not designed with everyone in mind, and showing up consistently in ways that are affirming, reliable, and human. You bring a trauma informed lens to your work, recognizing how past and ongoing experiences shape how individuals engage with care, and you respond with patience, transparency, and clarity.
Relational and Communicative
You are someone who builds strong, authentic relationships. You communicate with care and intention, adapting your approach to meet people where they are while maintaining clear boundaries and follow through. You are as comfortable sitting with complexity as you are moving toward action - able to listen deeply, assess needs, and help translate overwhelming systems into concrete next steps.
Resourceful and Solutions Oriented
You are resourceful and solutions oriented. When barriers arise, you stay engaged, curious, and persistent. You think critically about how to move forward, whether that means finding alternative pathways, advocating within systems, or helping a client take the next manageable step. You exercise sound judgment, balancing urgency with sustainability.
Organized and Accountable
You are organized and accountable in how you manage your work. You understand that strong documentation, timely follow up, and attention to detail are essential to providing high quality care. You are able to hold multiple priorities at once without losing sight of the people behind the work.
Equity Focused and Culturally Competent
You bring a demonstrated commitment to equity, inclusion, and belonging, particularly as it relates to LGBTQIA+ communities and individuals with intersecting marginalized identities. You understand how systemic inequities - across race, gender identity, sexual orientation, class, and other lived experiences - shape access to care and opportunity. You actively work to ensure that services are not only accessible, but affirming and responsive. You are willing to reflect on your own identities and experiences, understand how they show up in your work, and continuously grow in your ability to support others with humility and respect.
