What Is Care Coordination?

At Triangle Community Center, Care Coordination is here for LGBTQIA+ individuals and families of all ages - whether you’re navigating a major life transition, looking for affirming resources, or just need help getting started. Our Care Coordinators offer one-on-one support to help you access the services and solutions that matter most to you.

Whether you're a young adult exploring identity, a parent advocating for your child, or an older adult seeking connection or care, our team provides affirming, compassionate guidance every step of the way. We work with you to identify goals, reduce barriers, and connect you to community-based supports in a way that honors your lived experience.

How We Help

TCC’s Care Coordination program centers LGBTQIA+ needs, identities, and challenges. We recognize the barriers our community faces and are here to support you through:

  • Access to affirming mental health care and primary care

  • Gender-affirming care navigation and support

  • Housing stability, including emergency and transitional housing referrals

  • Help obtaining legal documents like name and gender marker changes

  • Support for coming out, family communication, and identity affirmation

  • School and education access for LGBTQIA+ youth and families

  • Employment support, job readiness, and workplace rights

  • Referrals for addiction recovery, harm reduction, and peer support

  • Support for parents and caregivers of LGBTQIA+ youth

  • Social connection opportunities to reduce isolation and foster belonging

When you work with a TCC Care Coordinator, you can expect:

1. Initial Interview – A welcoming conversation about your goals and concerns

2. Assessment – We’ll work together to understand your strengths, needs, and priorities

3. Support – Personalized, affirming guidance that puts your voice at the center

4. Referrals – Direct connection to local providers, services, and resources

5. Ongoing Follow-Up – Continued check-ins to support your progress and wellbeing
