Care Coordination
What Is Care Coordination?
At Triangle Community Center, Care Coordination is here for LGBTQIA+ individuals and families of all ages - whether you’re navigating a major life transition, looking for affirming resources, or just need help getting started. We work with you to identify goals, reduce barriers, and connect you to community-based supports in a way that honors your lived experience.
